Clear aligner therapy has transformed the field of orthodontics by providing a subtle and convenient alternative to conventional braces. However, to achieve the best possible results, additional support in the form of adjuncts is often necessary. Adjuncts refer to supplementary tools, techniques, or procedures that can boost the effectiveness and efficiency of clear aligner treatments.
Interproximal Reduction (IPR)
also known as tooth stripping, slenderizing, or enamel reduction—is a dental procedure where a small amount of enamel is removed from between the teeth to create space.
Why is it done?
IPR is most commonly used in orthodontic treatments, especially with clear aligners like Invisalign, to:
- Create room to straighten crowded teeth.
- Improve the fit or alignment of teeth.
- Reduce black triangles (spaces near the gum line between teeth).
- Enhance the bite and reduce the need for extractions.
How is it done?
orthodontist uses abrasive strips, disks, or burs to gently remove 0.2 to 0.5 mm of enamel from the sides of selected teeth. It’s usually painless because enamel doesn’t have nerves, though patients may feel some vibration or pressure.
Attachments
Attachments (sometimes called buttons) are tiny, tooth-colored shapes made of composite resin (the same material used for white fillings) that are bonded to certain teeth during aligner treatment.
Why do you need Attachments?
Aligners alone sometimes aren’t enough to grip and move teeth in complex ways. Attachments give the aligners extra grip or leverage to apply force more precisely, especially for:
- Rotating teeth
- Pulling teeth down (extrusion)
- Pushing teeth up (intrusion)
- Shifting teeth more than just a little
- Correcting bite issues
Elastic bands
Elastic bands, also known as rubber bands or interarch elastics, are often used as supportive tools during clear aligner treatment. These small bands are connected to specific points on the aligners and apply gentle, continuous pressure to help correct bite issues like overbite, underbite, or crossbite. By guiding the movement of the upper and lower jaws, elastics help improve occlusion and promote a more balanced and functional bite.
TADs (Temporary Anchorage Devices)
TADs (Temporary Anchorage Devices) in aligner treatment are tiny titanium screws or mini-implants that are placed in the jawbone to provide a fixed anchor point for moving teeth. They’re a powerful tool used in certain advanced orthodontic cases — including with clear aligners.
